(ANNUAL MEMBERSHIP)
We offer a discounted membership plan in order for our patients to maintain optimum oral health. It allows patients without a traditional insurance plan to obtain affordable dental care. It may be purchased individually or for an entire family. The member discount can be applied to most dental services provided by our doctors at Prime Dental Centers.

•$299 per Child 17 years old and younger
•$650 for a couple
•$1200 for a family of four
•Two Complimentary Exams
•Two Cleanings (Prophylaxis)*
•One set of Necessary X-Rays
•One Fluoride Treatment for child 17 years old and younger
•15-25% Discount on Most Dental Services**
•No Annual Maximum
•No Deductible
•No Preauthorization for Treatment Required
•Periodontal cleanings and fluoride treatment are not included.
